If you would like to become a client at Bayside, please fill out the forms provided on this page. They can be filled out as PDFs on your computer or printed and scanned back to us. We ask that you complete all of the new client forms and have your pet's previous medical records emailed to us before we schedule your appointment. This will give our medical staff the chance to review your pet's medical history and ensure that we are as thorough and efficient as possible in providing care to your furry friend. All forms and medical records should be emailed to: reception@baysidevetcare.com.
